Client:
Pennsylvania Governor´s Office of Health Care Reform
Challenge:
Support the design and facilitation of the statewide implementation of the Chronic Care Model.
Response:
Bailit advised the state on a strategy for improving chronic care in the Commonwealth using the Chronic Care Model. Bailit facilitated stakeholder work to develop a framework for the implementation, and supported the state, its newly created Chronic Care Commission, and the Commission’s committees in the operational design and implementation of the initiative through regional rollouts, beginning May 2008 in the Philadelphia region.

Challenge:
Develop a purchaser guide to the Patient-Centered Medical Home.
Response:
Bailit studied existing and developing medical home initiatives, interviewed employer purchasers, and solicited stakeholder input to guide the development of guide to educate employer purchasers regarding the Patient-Centered Medical Home, its potential benefits, and strategies for encouraging and supporting its development.


Client:
Massachusetts Executive Office of Health and Human Services
Challenge:
Facilitate the design and implementation of a multi-stakeholder Patient-Centered Medical Home initiative.
Response:
Bailit is advising the state and a multi-stakeholder council on the design and implementation of a public-private multi-payer medical home initiative. Work includes council meeting planning and facilitation, program design draft development, and technical consultation services.
